You may attach the form below or send it to our accounting team at accountsreceivable@alliedsupplycorp.com, remember, if submitting the Wisconsin form you must include your business tax registration number AND sign the form.
Because you won't qualify for sales tax exemption we will not be able to set up an customer account for you. The good news is we have customers in every state in the US, and on all but one continent, we can send information so you may buy our products from one of our customers near you.
